At Luff Orthodontics, our orthodontists, Dr. Cliff Luff and Dr. Richard Luff, and team are committed to helping all our patients receive excellent care. To schedule your Denali Kid Care orthodontic screening consultation in our office:

1. Print the Oral Health and Hygiene Assessment form linked on this page. You may also pick up a copy of the form in our office.
2. Take the Oral Health and Hygiene Assessment form to your dental provider for their signature.
3. Bring the completed form to our office. We will then schedule your assessment consultation appointment.

If your child’s orthodontic care in Wasilla, Alaska, is covered by Denali Kid Care, an Orthodontic Referral Oral Health and Hygiene Assessment needs to be performed by your general dentist before scheduling a visit with us.
